Home Hospice Support

If a friend or family member is placed on hospice, they may wish to remain at home and request some additional in-home care. The Caring Connection will help to keep your loved one comfortable, safe and in their familiar surroundings.  While the hospice program is a great resource, hospice teams are generally only able to provide a specific type of care.  Our Direct Care Workers will be there to supplement your loved one’s medical care with some additional non-medical home care during this difficult time.

With our team of trained professionals, you will have the peace of mind knowing your friend or family member is being cared for. Our staff will be there to provide any personal care needed, prepare meals, assist with feeding, toileting, and make sure the correct medications are taken on time. They will be there to provide the necessary compassionate care your loved one deserves.

Although providing the best quality care to your loved one is the main goal, the health and well-being of friends and family is also a priority.  Caring for someone on hospice can become physically and emotionally exhausting, especially if you are the primary caregiver.  Not only will our Direct Care Workers provide you with some much-needed time to yourself, they will be there to listen and provide support to you and the one you love.

When you feel the need for further care, please do not hesitate to contact our office.  Our Care Coordinator will meet with you to answer any questions you may have. At that time, we will assess your loved one’s needs and provide them with a customized plan of care.
